If you have not yet had a chance to place an order, you can click the links below to order online, download a printable order form and view proofs when they are made available. Feel free to mail the order form with payment to the address on the order form. Or if you prefer, you can email your order to
and use PayPal for payment.
For thise of you who have already placed your order, you have a choice when ordering. You can select the photo(s) that we use to fill your order. Just click the link below, select your favorite photo and email the filename you would like us to use to fill your order (e.g., FDRHockey-9). If we do not hear from you prior to the orders going to print, don't worry, we will choose the best photo of your player to fill your order.
I often recommend that you let us choose which photos look best for the design, but if you would rather choose that is OK too.
